A vet release form is designed to provide you with authorization to seek medical treatment for a pet in your care and to identify who is responsible for the cost of treatment.
Your key handling form should identify your procedures and policies related to managing your client keys. This form is normally only used by a pet care business that keeps keys on file.
A payment authorization form should detail your companies policies related to payments. If your business requires a credit card on file or if you process payments for your clients, add that information here.
